About The Position

The Registered Nurse is responsible for the comprehensive delivery of patient care, utilizing the nursing process of assessment, planning, implementation, and evaluation. This role involves directing and guiding patient education, as well as overseeing the activities of ancillary personnel, all while upholding professional nursing standards. The nurse is expected to understand and adhere to GCMC’s Compliance Standards, including the Corporate Compliance Policy, Code of Conduct, and Conflict of Interest Policy. Additionally, the position requires staying informed about all relevant federal, state, and GCMC regulations, laws, and policies as they evolve.

Responsibilities

  • Deliver patient care through the nursing process of assessment, planning, implementation, and evaluation.
  • Direct and guide patient teaching.
  • Direct and guide the activities of ancillary personnel while maintaining standards of professional nursing.
  • Understand and adhere to GCMC’s Compliance Standards, Code of Conduct, and Conflict of Interest Policy.
  • Keep abreast of all pertinent federal, state, and GCMC regulations, laws, and policies.
  • Perform patient assessment and document accurately in a timely manner per nursing and department policy.
  • Perform nursing admission assessment thoroughly and within timeframe, documenting accurately and completely.
  • Perform skin risk assessment accurately and reassessment per policy and protocol.
  • Perform pain assessment and reassessment per policy, utilizing hospital approved pain scale and documenting appropriately.
  • Monitor patient as appropriate for condition, status, and safety according to nursing department policy.
  • Monitor patient’s vital signs and systems per protocol and document on appropriate forms.
  • Recognize change in patient’s condition or status and report to charge nurse or physician as appropriate.
  • Call CAT team for patients who meet criteria.
  • Ensure patient safety is maintained in all aspects of patient care by following all policies related to National Patient Safety Goals.
  • Raise concern about patient care and utilize the Chain of Command as needed to prevent patient harm.
  • Perform safe administration and complete documentation of medications, IV fluids, and blood products in accordance with nursing policy.
  • Follow medication administration policy using e-Mar, bar coding, and scanning of medications, aiming for at least an 85% scan rate.
  • Insert IVs under aseptic technique and follow IV policy by changing site, tubing, and dressing, documenting accordingly.
  • Always follow hospital policy when administering blood products.
  • Maintain security of electronic signature with Omnicell and account for all narcotics removed from automated dispensing system through proper waste/return administration documentation.
  • Assess and address educational needs of patients and families, including disease state, care, and treatment.
  • Document educational needs and distribute appropriate teaching sheets.
  • Include evaluation of learner’s understanding post-teaching.
  • Contribute to or initiate discharge plan upon day of admission and participate in interdisciplinary planning.
  • Complete all pre-discharge checklist items.
  • Always give patient correct forms and discharge instructions by using 2 patient identifiers.
  • Provide patient a safe discharge and ensure appropriate follow-up information and community resources are in place.
  • Assist providers in performing procedures and treatments per unit, department specific, and hospital policy.
  • Assist physicians/providers with bedside procedures and remain competent with unit-specific procedures.
  • Perform read back of critical values and telephone orders when interacting with physicians as warranted.
  • Use SBAR when giving physicians report on change in patient condition.
  • Demonstrate knowledge and ability to use equipment safely and effectively.
  • Utilize hospital equipment per policy (e.g., guardrails for IV pumps, alarm settings).
  • Use patient lift equipment per policy at all times.
  • Demonstrate the ability to perform nursing treatments and procedures safely and effectively.
  • Conduct Hourly Rounds on patients using 4 “P’s” and document on appropriate forms.
  • Regularly use patient room white boards to communicate information to patients and families.
  • Demonstrate working knowledge and understanding of emergency preparedness plans and operations.
  • Be aware of and actively participate in emergency preparedness procedures and operations, including Code Blue, Green, Pink, Orange, White, and Silver drills and actual events.
  • Completely and accurately document patient care activities and response in a timely manner in accordance with department and hospital policy.
  • Document on a regular, real-time basis all patient care activities in accordance with hospital policy.
  • Promote professional nursing through participation in PI/QI, customer satisfaction, community involvement, committee activities, and nursing continuing education.
  • Actively sit on and attend at least 1 team or committee, or participate in unit PI activity or department team or projects.
  • Attend at least 75% of all staff meetings.
  • Attend at least 1 clinical update and all mandatory in-services.
  • Participate in unit specific or hospital wide PI/QI activity.
  • Provide safe and effective patient care in accordance with hospital Infection Control policy and procedures.
  • Wear appropriate PPE as warranted, follow hand hygiene protocol, and follow all isolation procedures.
  • Utilize the nursing process to identify patient needs and problems through assessment, implement nursing interventions based on nursing diagnosis, develop individualized patient care plans, and provide appropriate discharge instructions.
  • Initiate individualized patient plan of care for all admissions and edit Plan of Care on a regular basis per policy.
  • Document interventions every shift based on nursing diagnosis.
  • Assist and provide for participation in clinical learning experiences.
  • Participate in learning experiences for orientees, nursing and ancillary staff, and students by serving as a preceptor.
  • Participate in Nursing Grand Rounds at least quarterly by either attending a session or assisting in preparing for unit topic.
  • Assure safe patient handoff through effective communication of the treatment status of each patient.
  • Participate in interdisciplinary care/discharge planning.
  • Use SBAR for patient handoffs and Ticket to Ride to communicate important information to other departments.
  • Perform other duties as assigned.
