Serves as a patient advocate/liaison, overseeing the continuum of care for the patient from hospital admission to discharge. The RN Care Manager utilizes clinical skills to assess patient needs and develop an individualized discharge plan of care. The RN Care Manager will take a proactive role by early intervention and collaborative practice with staff nurses, physicians, ancillary services, payer sources, and outside agencies assuring appropriate use of resources while focusing on positive outcomes. Works with the UR team to assure patient is being treated in the most appropriate level of care; documents these reviews in the EMR. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in their area. Why Holland Hospital?
