About The Position

The Discharge Planner plays a critical role in coordinating and facilitating the safe and efficient transition of patients from healthcare facilities to appropriate post-acute care settings. This position ensures that each patient’s discharge plan is comprehensive, addressing medical, emotional, and social needs to promote optimal recovery and prevent readmissions. The Discharge Planner collaborates closely with multidisciplinary teams, including medical staff, social workers, and external care providers, to develop individualized care plans that comply with Medicaid and other regulatory requirements. By assessing patient needs related to dialysis, long-term care, and mental health, the Discharge Planner ensures continuity of care and access to necessary resources. Ultimately, this role supports patients and their families through education and advocacy, fostering emotional stability and confidence during the discharge process.

Requirements

  • Bachelor’s degree in Social Work, Nursing, Healthcare Administration, or a related field.
  • Experience in discharge planning, case management, or social services within a healthcare setting.
  • Knowledge of Medicaid policies and procedures relevant to discharge planning.
  • Familiarity with long-term care services, dialysis treatment protocols, and mental health resources.
  • Strong communication and organizational skills with the ability to manage multiple cases simultaneously.

Nice To Haves

  • Master’s degree in Social Work, Nursing, or Healthcare Administration.
  • Certification in Case Management (CCM) or Discharge Planning.
  • Experience working with diverse patient populations and complex care needs.
  • Training in emotional support techniques and crisis intervention.
  • Proficiency with electronic health records (EHR) and discharge planning software.

Responsibilities

  • Conduct thorough assessments of patients’ medical, social, and emotional needs to develop effective discharge plans.
  • Coordinate with healthcare providers, social services, and community resources to arrange appropriate post-discharge care, including long-term care and dialysis services.
  • Ensure compliance with Medicaid guidelines and other insurance requirements during the discharge planning process.
  • Provide education and support to patients and families regarding discharge instructions, follow-up care, and available community resources.
  • Maintain detailed documentation of discharge plans, patient progress, and communication with all stakeholders.
  • Monitor and address barriers to discharge, including mental health concerns and social determinants of health.
  • Participate in interdisciplinary team meetings to discuss patient care plans and update discharge strategies as needed.
