How you’ll make an impact in this role Champion individualized care by implementing and monitoring patient-centered care plans, ensuring every intervention is tailored to the unique clinical and personal needs of the patient. Serve as the heartbeat of the care team by coordinating communication across all disciplines, ensuring a seamless, collaborative experience for both the patient and their support network. Act as a proactive patient advocate by continuously assessing health status, documenting clinical changes, and relaying vital information to ensure the entire team is aligned in delivering high-quality care. Drive continuity of care by leading discharge planning efforts, helping patients and their families confidently transition from our facility to their next stage of healing. Execute high-standard clinical support by accurately interpreting and carrying out orders, serving as a reliable partner in the execution of the patient’s overall treatment strategy.

Requirements

  • Advanced Life Support obtained within 6 Months (180 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
