About The Position

Utilizing your nursing expertise to provide compassionate support for older adults with chronic conditions (e.g., Diabetes, COPD, heart disease), demonstrating a commitment to patient needs and well-being.
Facilitating improved patient navigation within the health system.
Analyzing patient data, specifically tracking admissions and readmissions.
Collaborating with managers and primary care providers to identify high-risk patients.
Attending required internal team meetings.

Requirements

• High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
• Quality Professional preferred.
• Risk Management preferred.

Nice To Haves

• RN or LPN license.
• Experience with older adults and chronic disease management.
