How you’ll make an impact in this role Lead personalized care plans by monitoring patient progress to ensure high-quality clinical outcomes and safety. Coordinate multidisciplinary teams as the primary point of contact to provide a seamless, holistic patient experience. Execute physician and nursing orders with precision, ensuring accurate treatment and high-standard clinical care. Champion the discharge process by collaborating with the care team to ensure patients transition home safely and successfully.

Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
  • Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.
