Requirements

• Current Florida RN license (or eligible).
• Strong communication, critical thinking, and organizational skills.
• 1 year Case Management Experience
• Cardiopulmonary Resuscitation Certification (CPR) Required
• Licensed Registered Nurse Required

Nice To Haves

• Previous experience in case management, discharge planning, or utilization review preferred.
• Bachelor's Degree - Nursing

Responsibilities

• Coordinate care for hospitalized patients.
• Complete high-risk screenings and patient assessments.
• Plan and document post-acute care services.
• Collaborate with physicians, nurses, and multidisciplinary team members to ensure quality outcomes.
