About The Position

Provides case management and clinical support for patients in specialty services. Coordinates care across providers, referrals, and specialty clinics to ensure timely, efficient, and high-quality outcomes. Serves as the primary clinical contact for patient care coordination.

Requirements

• Current, full, unrestricted Registered Nurse (RN) license in any U.S. state.
• Basic Life Support (BLS) certification.
• Clinical Competency required within 3 months of hire and every three years.
• High school diploma or equivalent.
• Knowledge of the nursing process and ability to apply it in a clinical setting.
• Understanding of customer service principles and infection control standards.
• Ability to work in a fast-paced, multidisciplinary healthcare environment.
• Patient assessment, anticipation of needs, and data collection.
• Oral and written communication.
• Critical thinking and problem-solving.
• Ability to multi-task and work independently while collaborating with team members.
• Willingness to teach and seek assistance when encountering new or challenging situations.
• Practice evidence-based nursing and shared governance principles.
• Regular talking, hearing, standing, walking, sitting, and use of hands/arms.
• Must lift and/or move up to 50 lbs

Nice To Haves

• Bachelor’s degree in Nursing preferred.

Responsibilities

• Patient Care & Case Management: Actively manage patient care and collaborate with medical providers and interdisciplinary teams to establish individualized care goals.
• Perform staff nursing duties including assessment, documentation, education, and administration of medications and therapies.
• Coordinate patient referrals to specialty clinics, reviewing records to ensure completeness.
• Track referral progress and outcomes, providing periodic status reports to providers.
• Identify delays and obstacles to care and proactively address issues to improve the patient care process.
• Communications & Teamwork: Collaborate effectively with clinical staff, departments, and interdisciplinary teams.
• Participate in clinic and department improvement efforts.
• Perform other duties or special projects as assigned to provide comprehensive healthcare services.
