About The Position
We are seeking an experienced Registered Nurse Case Manager to support care coordination in an acute care setting. The ideal candidate will have strong experience managing patient care plans, discharge planning, and collaborating with multidisciplinary teams to ensure optimal patient outcomes.
Requirements
- Minimum 2 years of acute care Case Management experience
- Experience in care coordination and discharge planning
- Prior travel experience required
- Ability to manage a patient ratio of approximately 1:25
- Strong organizational and communication skills
- Active Registered Nurse (RN) license – California (required)
- License must be active by start date (pending accepted at submission)
- Strong clinical judgment and problem-solving skills
Responsibilities
- Coordinate and manage patient care plans across the continuum of care
- Perform discharge planning and utilization review
- Collaborate with physicians, nurses, and allied health professionals
- Ensure appropriate resource utilization and patient advocacy
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely documentation
