Role Overview As an RN Case Manager, you coordinate and deliver high‑quality, patient‑centered home health care. Your responsibilities include: Providing direct care to patients with acute and chronic conditions in their homes Documenting assessments and validating patient needs Promoting patient autonomy and helping them adapt to changing health situations Applying strong critical‑thinking skills to ensure safety and comfort Preventing adverse events and supporting positive health outcomes This role is ideal for a nurse who values independence, strong patient relationships, and the ability to make a meaningful impact every day.

Requirements

  • Current RN license
