Requirements

Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
BLS Provider preferred. American Heart Association or American Red Cross accepted.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

Previous charge or supervisory experience preferred.
Previous experience in a hospital setting preferred.

Responsibilities

Coordinate the interdisciplinary plan of care from admission through discharge, ensuring seamless transitions and consistent, high-quality patient care.
Monitor patient progress through ongoing assessments and medical record review to ensure care plans remain appropriate and goal-focused.
Advocate for patients by securing necessary resources and removing barriers that may delay treatment, discharge, or recovery.
Collaborate with physicians, nurses, and interdisciplinary team members to ensure timely implementation of the health plan and continuity of care.
