Requirements

  • Proven experience in home health care, hospital or rehabilitation is preferred.
  • understanding of trauma protocols and emergency medicine procedures.
  • Hands-on experience in hospital medicine, medical-surgical units, or emergency room settings demonstrating advanced patient assessment skills.
  • Background in post-acute care coordination including discharge planning for complex cases involving geriatrics or behavioral health populations.
  • Familiarity with managed care principles and utilization management processes to optimize resource use without compromising quality of care.
  • Knowledge of physiology and medical terminology essential for accurate case documentation and diagnostic evaluation.
  • Ability to navigate electronic health records efficiently while maintaining strict compliance with HIPAA standards.
  • Ability to travel within the State of Maine and must have an Active RN Maine License.

Nice To Haves

  • Experience working within hospital or outpatient clinics—particularly in pediatrics (PICU), Geriatrics, or outpatient settings—is highly desirable.

Responsibilities

  • Develop personalized care plans that incorporate discharge planning, and creating case plans for in home health care.
  • Collaborate with multidisciplinary teams—including operations, rehab facilities, therapists, social workers, and other healthcare professionals—to coordinate patient treatment plans.
  • Manage utilization through effective case management practices, ensuring appropriate level of care while adhering to HIPAA regulations.
  • Facilitate intake processes for inpatient and outpatient services, including nursing homes, rehab and skilled care facilities.
  • Ensure compliance with policies, infection control standards—particularly in infectious disease care—and safety protocols across all patient interactions.
