As a Full-Time Registered Nurse Case Manager at Benevolent Cleveland Hospice Operations, LLC, your daily responsibilities will involve assessing patients' needs through comprehensive evaluations and developing personalized care plans that reflect their preferences and requirements. You will coordinate care with a multidisciplinary team, ensuring seamless communication among healthcare providers, patients, and families. Daily tasks will include monitoring patient progress, adapting care plans as necessary, and providing education and support to patients and their families regarding treatment options and palliative care strategies. You will document all patient interactions and care plans meticulously to maintain compliance and uphold our commitment to excellence. Additionally, you will participate in regular team meetings to discuss patient cases and best practices, fostering a culture of continuous improvement and innovation. Requirements

  • Current RN license in the state of practice
  • Associate's or Bachelor's degree in Nursing
  • Experience in hospice or palliative care nursing for a minimum of two years
  • Proficiency in pain and symptom management
  • Crisis intervention skills
  • Deep understanding of Medicare, Medicaid, and hospice regulations
  • Exceptional communication skills
  • Demonstrated critical thinking and decision-making abilities
  • Experience in collaborative teamwork within an interdisciplinary hospice team (IDT)
  • Familiarity with electronic medical records (EMR) systems for accurate documentation
  • Emotional resilience
  • Strong patient advocacy skills
