Registered Nurse - Case Manager (Purchased/Referred Care)

Requirements
- At least 3 years of experience as an Registered Nurse, with case management and Purchased/Referred Care (or similar) experience. Experience with similar programs (TriCare, Medicare, etc.) is transferrable.
- Permanent, current, full and unrestricted license to practice a Registered Nurse in a state, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
- Ability to obtain an IHS public trust clearance.
Responsibilities
- Providing specialized case management and care coordination for patients with special health care needs.
- Reviewing and monitoring outpatient consultations and referrals for medical necessity, cost effectiveness, and appropriateness.
- Collaborating with PRC staff, providers, and external facilities to ensure care aligns with CMS criteria.
- Maintaining statistical data on consults, monitoring deferred referrals, and supporting PRC team continuous quality improvement.
