About The Position

The Registered Nurse Case Manager is responsible for supervising, evaluating, and directing the care and supervision of the bio-psycho-social needs of residents in the licensed Residential Care Facility for the Chronically Ill.

Requirements

  • Current California license as a Registered Nurse
  • Must pass thorough live scan - criminal background check
  • CPR/First Aid Certification
  • Must be culturally sensitive and non-judgmental
  • Knowledge and sensitivity regarding issues of homelessness and/or HIV/AIDS
  • Basic knowledge of the care needs of HIV individuals
  • Ability to work with diverse populations in a caring, sensitive, and non-judgmental manner
  • Ability to walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis.
  • Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead.
  • Ability to operate computer, fax and telephone.
  • Ability to lift up to 25 lbs.

Nice To Haves

  • 5 Years of Experience Preferred
  • Bilingual (English/Spanish) is preferred.

Responsibilities

  • Evaluate each new Qualified resident and complete a baseline physical, psychosocial, and home care needs assessment.
  • Provide input on the coordination and/or the provisions of the services specified in the Individual Service Plan.
  • Obtain a completed medical assessment form and other required information from the resident's physician.
  • Obtain medical orders and facilitate home health and hospice referrals to outside agencies as indicated.
  • Provide medically related counseling to residents regarding AIDS and HIV disease, current treatment, available resources and probable outcomes.
  • Act as a resource to staff for medical problem solving.
  • Document all contact with residents and team members concerning individuals in the resident's files.
  • Administer medications upon a physician's order; evaluate requests for PRN medications, fill medication cassettes and dispose of controlled substance medications according to the law.
  • Educate residents on life skills pertaining to both medical and emotional stabilization
  • Attend a monthly team meeting to review each resident's progress. Update each resident's Service Plan accordingly
  • Perform wellness checks with all residents during shift
  • Review files for accuracy
  • Provide case management for internal and external referrals.
  • Provide in-service education to staff regarding the medical care and medical supervision of persons with HIV/AIDS and on universal precautions.
  • Participate and implement improvements through quality management process.
  • Work with administration to ensure conditions of licensing and contract requirements are successfully implemented.
  • Monitor provision of care by Certified Nursing Assistants.
  • Monitor implementation of service plans by Residential Care team.
  • Assist in coordinating HIPAA compliance activities.
  • Communicate and coordinate needs of residential care program to the leadership team.
  • Case Watch Training, HMIS training
  • Able to work as needed at other programs under Alegria or as needed
  • Other duties as assigned.
