About The Position

We are seeking a compassionate, skilled, and experienced Hospice RN Case Manager to join our dedicated hospice care team in Milledgeville. This full-time leadership role is essential to ensuring that patients receive the highest standard of end-of-life care in accordance with our mission of dignity, comfort, and compassion. The RN Supervisor will not only deliver direct patient care but also serve as a clinical leader, managing day-to-day hospice field operations, supporting and supervising staff, and coordinating care with interdisciplinary teams. Ideal candidates are empathetic clinicians with hospice experience, a strong sense of initiative, and a passion for mentoring others in a supportive, patient-centered environment.

Requirements

  • Active RN license in the state of Georgia (required)
  • Minimum of 2 years of hospice or home health experience (preferred)
  • Strong clinical knowledge and assessment skills
  • Proficient with EMR systems and electronic documentation
  • Exceptional communication, organization, and leadership abilities
  • Compassionate, calm under pressure, and highly adaptable

Nice To Haves

  • Prior supervisory or team lead experience is a plus

Responsibilities

  • Oversee and support day-to-day clinical operations across the hospice team
  • Supervise LPNs and CNAs, ensuring high standards of clinical care and professionalism
  • Conduct supervisory field visits to evaluate care delivery and staff performance
  • Mentor and guide staff nurses in clinical decision-making, patient communication, and documentation
  • Perform skilled nursing visits, including start-of-care (admissions), routine, and PRN visits
  • Provide comprehensive head-to-toe assessments and pain/symptom management
  • Develop, update, and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG)
  • Educate patients and caregivers on disease progression, medications, and hospice philosophy
  • Respond to changes in patient condition with clinical interventions and communication with the medical director
  • Lead the admission process by completing initial assessments, obtaining consents, and setting care expectations
  • Coordinate DME delivery, medication orders, and first visits by all disciplines
  • Be available for acute needs and symptom management as they arise
  • Attend and actively participate in weekly IDG meetings to review and coordinate ongoing care plans
  • Collaborate with physicians, social workers, chaplains, and therapists to ensure holistic care
  • Act as a liaison between families, team members, and the organization to ensure clear communication
  • Ensure documentation is accurate, timely, and compliant with Medicare and agency standards
  • Participate in QAPI (Quality Assurance and Performance Improvement) activities
  • Monitor clinical metrics, patient outcomes, and staff adherence to protocols
  • Implement corrective actions or performance improvement plans when needed
