About The Position

The RN clinical assessor requires advanced knowledge and understanding of the services and benefits available for Veterans and Caregivers from federal, state and private sector resources. The clinical assessor participates in face-to-face or virtual contact with the Caregiver and Veteran to assess function and the need for personal care services. Face-to-face assessments occur mostly within the veteran's home and would require travel in a government car. The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. Requirements

  • Advanced knowledge and understanding of the services and benefits available for Veterans and Caregivers from federal, state and private sector resources.
  • Licensure: 1 full and unrestricted license from any US State or territory

Responsibilities

  • Driving a government vehicle is required for local travel.
  • Responsible for implementing the Caregiver Support program specific to his/her area of assignment and ensures compliance and collaboration with local facility and the Veterans Administration Central Office Caregiver Support Program initiatives.
  • Conducts initial and annual home visits to Family Caregivers applying and approved in the program, and coordinates/integrates findings from the assessments into learning activities.
  • Collaborates with the facility Caregiver Support Program coordinator to implement Caregiver support services and benefits.
  • Provides referrals or consults, or in some cases directs the admission of a Veteran to other VA programs including the Respite Program, Community Health Care Coordination Program, Hospice, Home Based Primary Care, Community Nursing Home, and other programs.
  • Regular symptom management with in-depth body systems assessments.
  • Facilitation of Homecare referrals and recommendations to support ongoing Veteran needs
  • Patient and Caregiver education and support.
