About The Position

Overture Home Care is seeking a dedicated and professional RN Nurse Assessor to join our growing team. This position plays a vital role in ensuring high-quality client care by conducting admission assessments, reassessments, and supervisory visits, while also supporting our caregivers with training and guidance. The Nurse Assessor helps maintain continuity of care, fosters strong client relationships, and represents our agency with professionalism and compassion. This role provides the opportunity to make a meaningful impact on clients and their families while also supporting and mentoring our caregiving staff. Requirements

• RN degree (AD, BSN, or MSN) with active license in good standing (no suspensions or disciplinary actions).
• Valid driver’s license, reliable transportation, and clean driving record.
• Strong critical thinking, time management, and organizational skills.
• Proficiency with technology and accurate documentation.

Nice To Haves

• Home Health or Hospice experience preferred.

Responsibilities

• Perform in-home admission assessments for new clients and reassessments following hospitalizations, rehab stays, or condition changes.
• Update Plans of Care accurately and in a timely manner to ensure proper coordination of services.
• Conduct supervisory visits, caregiver introductions, and training for RN-delegated tasks.
• Participate in marketing, business development, and community events as needed.
• Follow up on client discharges and document reasons for ending services.
• Ensure compliance with agency policies, state regulations, and client rights.
