Position Summary: The RN Field Assessor is responsible for face to face assessing with members to evaluate the medical needs of the member to facilitate the member’s overall wellness and help them obtain the services they need to thrive. Addressing requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program and MLTSS program enrollment. Minimum of 20 assessments per week dependent on need, may be more. Assessments take into account information from various sources to address all conditions including co-morbid and multiple diagnoses that impact functionality. Assessments include the member’s level of work capacity and related restrictions/limitations. Using a holistic approach assess the need for a referral to clinical resources for assistance in determining functionality. Consults with supervisor and others on team in overcoming barriers in meeting goals and objectives. Utilizes case management processes in compliance with regulatory and company policies and procedures. Utilizes interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation. The RN Assessor will also be expected to mentor new hires on the team, once, a level of proficiency has been attained in their role. Work at Home Requirements: Candidate will work from home (WAH) out of their residence in NJ Have reliable phone and hard-wired internet service. Work schedules are determined by the Leader based on business needs and are not affected by the work model or location. Mon-Fri 8-5 EST Colleagues must be reachable via phone, videoconference, and email during work hours (8 Hours per Day) as directed by their Leader. Personal responsibilities, including dependent care, must not interfere with work duties. Dependent care during work hours is generally not allowed. Be secure and private to maintain confidentiality (HIPAA compliant). Meet any additional reasonable requirements set by the Leader or Company. Field Requirements: A field-based Colleague regularly works in multiple company locations. Those locations may include company locations, client locations, facilities (nursing home, rehabilitation centers, etc.), member homes, and/or community locations. Unlike office-based and hybrid Colleagues who generally work in the same company location during their work week, field-based Colleagues
