Requirements

  • Active, unrestricted RN license
  • Residence in Essex County, New Jersey.
  • 2+ years clinical experience (hospital, home health, or ambulatory care).
  • Reliable transportation and willingness to travel up to 75%.
  • Comfortable working with diverse populations in various settings.
  • Strong attention to detail, organizational skills, and ability to work independently.
  • Computer proficiency and experience handling medical records.
  • Ability to work Monday–Friday, 8:00 AM–5:00 PM EST.

Nice To Haves

  • Assessment, case management, or discharge planning experience.
  • Managed care or crisis intervention background.
  • NJ Choice certification (or willingness to obtain).
  • Strong interviewing and communication skills.
  • Proficiency with MS Office Suite and EMR systems.

Responsibilities

  • Conduct face-to-face assessments with members to evaluate medical needs and overall wellness.
  • Help members access vital services such as: Adult & Pediatric Medical Daycare Personal Care Assistance Nursing Facility Custodial Care Personal Preference Program MLTSS Program Enrollment
  • Complete 20+ assessments per week (based on need) using a holistic approach that considers co-morbidities, multiple diagnoses, and functional limitations.
  • Collaborate with your team and supervisor to overcome barriers and achieve goals.
  • Use your interviewing skills to engage members and uncover their health needs.
  • Mentor new team members once you’ve mastered your role.
  • Operate at full capacity by month four with comprehensive training and support.
