Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Ambulatory Nurse Care Coordinator works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting. S/he will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary. The Clinical Nurse II will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely care received in clinic as well as coordination of care. In addition, the Clinical Nurse II will assist in the development of patient care standards and practices. The Pediatric Allergy & Immunology Nurse Care Coordinator is responsible for coordinating outpatient services and needs for patients and providers within the Pediatric Allergy & Immunology Ambulatory Clinic. His/Her responsibilities will include assisting with insurance authorizations, providing and coordinating procedural support, triage patients for priority and visit type in collaboration with scheduling, assisting with medication management needs, providing patient teaching and assisting in the overall coordination of timely patient care. The nurse should be competent in providing care to patients with urgent and severe allergic reactions. This position requires flexibility to orient and work at all UCSF Medical Center locations.
