They are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. We offer education and post-hospitalization care as necessary. Requirements

Associates degree in nursing or Diploma in Nursing Required, Bachelor Degree in Nursing preferred
Florida RN license and/or approved multistate RN license required. Compact license holders have 60 days to obtain the Florida license after relocation.
BLS-AHA Provider issued cardrequiredwithin30 days of start
ACLS required
Acute care experience with cardiac monitoring required

Responsibilities

Demonstrate knowledge and application of the nursing process.
Support, maintain and provide an environment which is supportive of quality patient care through effective use of resources and personnel.
Serve as the advocate for the patient and family with respect to the patient's right to privacy by protecting confidential information.
Plan and implement patient care and teaching based on a total assessment and the age of the patient.
Evaluate and document patient response; monitor and recognize changes in physiological and psychosocial status; take appropriate action based on assessment of the patient.
Practice, develop and work with bed management to meet the goals of patient placement.
