About The Position

Lead with purpose—drive high-quality, integrated care as a Regional Nurse Director shaping the future of Primary Care Health Home services. This is a full-time, benefit eligible position. The schedule is Monday - Friday 8:00am - 4:00pm. This candidate will travel and cover locations in the Eastern, Southern and Jefferson Regions. The Regional Nurse Director of Primary Care Health Home (PCHH) is a key member of the Health Home Leadership Team, helping guide strategy, strengthen partnerships, and ensure consistent, high-quality care delivery across the region. This role provides clinical leadership and supervision to Nurse Care Managers (NCMs), while supporting integrated, person-centered care that promotes wellness and improves outcomes in the communities we serve. This is a great opportunity for someone who is passionate about leading clinical teams, advancing integrated care, and using data and collaboration to improve health outcomes across diverse populations.

Requirements

  • Associates degree in nursing required
  • RN licensed in MO required

Nice To Haves

  • Bachelor’s degree in nursing preferred
  • At least two (2) years of clinical supervisory experience preferred
  • Experience in the primary care or behavioral health field preferred

Responsibilities

  • Provide clinical leadership, supervision, and coaching to a regional team of Nurse Care Managers
  • Support onboarding, training, and ongoing competency development of nursing staff
  • Ensure consistent implementation of Health Home policies, procedures, and care standards
  • Partner with multidisciplinary teams to strengthen care coordination and whole-person care delivery
  • Monitor program performance through data, reports, and dashboards to drive quality improvement
  • Promote integration of physical health, behavioral health, and wellness initiatives
  • Maintain compliance with state Health Home guidelines and organizational expectations
  • Support optimal caseload management, enrollment, and staffing across the region
  • Facilitate regular team meetings, site visits, and communication to ensure alignment and engagement
  • Collaborate with leadership to identify opportunities for process improvement and program growth
  • Provide guidance on documentation standards and support staff in utilizing data systems effectively
  • Assist with operational needs including supply management, workflow optimization, and special projects
