About The Position

The Regional Manager (RM) is a registered nurse who provides day-to-day support to direct care team members (RNs, LPN/LVNs, CNAs, Therapists, etc.) including supervision and oversight of patient’s plan of care, achievement of both clinical and administrative outcomes, and professional and supportive services to clients in a safe, cost-effective manner. The Regional Manager is responsible for the coordination of care during the home health benefit period, staff engagement and communication, safety, and training of staff. To operationally lead and develop an amazing team of compassionate and dedicated healthcare providers. Report to: The Associate Director of Nursing Regions Covered: San Antonio, Austin, & New Braunfels, TX This is a Remote Position, living in Region is preferred. Occasional travel to Dallas Corporation Office for meetings.

Requirements

  • Current State License as a Registered Nurse
  • 2 years of home care experience as a registered nurse in a Home Health or Hospice environment
  • 1 year of supervisory experience required
  • Advanced nursing skills as defined by accepted nursing standards, including ability to manage patient care episodes, changes to plan of care, clinical evaluations, and recommendations
  • Ability to set objectives, assess staff competencies, allocate work, facilitate team alignment, and create a motivating, safe working environment
  • Ability to provide constructive feedback and develop both clinical quality, core competencies, and productivity with direct reports
  • Ability to seek patient feedback, advocate for patients, understanding patient needs, and managing patient expectations
  • Quickly become proficient in enterprise applications (HC Net)
  • Ability to convey information and opinions, listening to others and checking for understanding, responding in appropriate message and tone
  • Ability to develop trusted relationships and networks, leverage personal networks, and relate across both supervisory and leadership levels
  • Valid driver’s license and reliable transportation to perform job tasks

Responsibilities

  • Knows Policy and Procedure of the Agency and effectively utilizes them to enhance performance and continuity of care; create policy if needed and get approval from ADON/HR
  • RN to collaborate with clinical team members and oversee the day-to-day operations of the regional staff
  • Provides regional coverage in the field as needed to promote growth and assist with limited staffing
  • Supports and promotes quality patient care, providing support to each employee.
  • Ensures adequate supervision by regularly re-evaluating the needs of the patient and making visits in the patient’s home when necessary
  • Observes safety and infection control policy and procedures including exposure control plan and OSHA requirements and assist with implementation of these policies
  • Assures accuracy of all OASIS assessments, including but not limited to completion of questions, ensure all areas correlate with diagnosis. Determining the episode of care and frequency of visits required for a patient and coordinating the level of staff person required to provide services. Review, sign and fax 485’s to MD
  • Review, sign and fax all verbal orders to MD
  • Functions as a team leader in the provision of implementing the plan of care, serving as liaison between the patient and all disciplines.
  • Coordinates/supervises care with the patient care team and reports to the physician reactions to treatments and changes in the patient’s physical condition
  • Supervises appropriate teaching, counseling and supervision of the patient care as relevant to the plan of care
  • Facilitates in service training programs, assists with timely client case conferences, and participates in professional staff meetings and provides in-service education as needed
  • Ensures that state and federal laws and regulations are followed that relate to patient care
  • Receives all regional patient complaints and assist in resolutions
  • Daily operational oversight of area Home Health team and collaboration with multi departments within the business line
  • Direct oversight of Clinical outcomes and KPIs including but not limited to, productivity, utilization, missed visits, non-admits, timeliness of documentation, incomplete visits, chart audit reviews and other applicable metrics driven to ensure quality patient care is provided
  • Delegates as necessary for the efficient operation of the specific region and all other duties as assigned
  • Serves as mentor and local engagement leader for care team members to ensure care team members relate to the company and each other.
