About The Position

Purpose of Position: Responsible for outreaching new referrals for S.T.A.R. peer center and counseling services. Contact the member within 48 business hours to schedule to meet at the members assigned facility and complete an intake. Provides professional, warm, and inviting engagement during outreach to schedule enrollment appointments for new members. Ensures accurate, complete, and quality enrollment paperwork that is individualized, member-centered, strength-based, inclusive of family and/or natural supports, culturally and linguistically appropriate, clinically sound, reflects the needs and goals of the individual member, supports the development of a service plan which effectively meets the member’s/family’s needs, and results in improved health outcomes. Completes initial outcomes measurement tools during enrollment, and works with the center receptionist and site management to guarantee outcomes measurement tools are completed quarterly. Serves as an additional center contact to schedule counseling sessions for members approved for counseling as needed. This position also ensures compliance with the S.T.A.R. Assessment & Treatment Documentation Process by maintaining timely communication with case managers, coordinating assessments and intakes, and ensuring all required documentation is completed before member activation.

Requirements

  • Must be able to be credentialed as a Behavioral Health Technician (BHT) within 90 days of hire:
  • High School/GED (Required)
  • 1 year at STAR OR 2 Years elsewhere

Responsibilities

  • Stationed at Central and travels to other facilities if needed.
  • Coordinates rides to other facilities and notifies management for new members.
  • Visit clinics to promote STAR services as assigned.
  • After receiving new member information, contact the member within 48 business hours to schedule to meet at the members assigned facility and complete an intake.
  • Attempt contact three times consecutively before moving on to the next referral. If no response, document as “no contact” and retry after 48 business hours.
  • Conduct outreach and document all contact attempts in Credible using quick notes and the intake tracker.
  • Work with Clinical Director or Counselor to schedule assessments and treatment plans when necessary.
  • For new members, ensure activation only occurs once valid assessments and ISPs are on file.
  • Clearly communicate with members about missing documents or delays in activation and provide grievance information as appropriate.
  • Maintain a professional and welcoming approach as the first point of contact for new members, setting the tone for a positive recovery experience.
  • Utilize the provided outreach script when contacting members to schedule intakes.
  • Utilize outlook for emails, calendar, and Microsoft teams communication as directed
  • Attends staff meetings and required training classes.
  • Must attend required live clinical oversight training as directed to maintain clinical supervision.
