JOB SUMMARY The Chief Regional Medical Officer (CRMO) will have overall responsibility for the medical component of Ennoble Care's home-based primary and palliative care programs within their designated multi-state region. The CRMO ensures high-quality, evidence-based care; supports clinical operations; partners closely with regional operational leaders; and maintains accountability for clinical outcomes, quality, compliance, patient safety, and provider experience. This role provides oversight, mentorship, and performance management for an interdisciplinary team of Nurse Practitioners (NPs), Physician Assistants (PAs), collaborating physicians, care coordinators, and other team members, in the delivery of primary care to homebound and medically complex patients. This executive leadership role combines clinical practice with strategic medical oversight, serving as the primary medical resource for interdisciplinary teams across the region, and ensures the provision of high quality, effective, and patient-centered care. This position constitutes a full-time (1.0 FTE) salaried role comprising three integrated functions: (1) independent clinical physician; (2) House Calls medical director (mobile primary care & palliative); and (3) Regional Leadership and Administration. Clinical practice will constitute approximately 60% of the CRMO's time, with medical director and administrative duties comprising approximately 40%, balanced to not exceed a full-time work schedule in any given week. REGIONAL SCOPE The CRMO will provide medical leadership and oversight across multiple states within their designated region. The CRMO will maintain appropriate medical licensure in all states within their region of responsibility.

Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
  • Board Certification by the American Board of Internal Medicine, American Board of Family Medicine, or equivalent specialty board
  • Active, unrestricted medical license in good standing in all states within the designated region (or ability to obtain within 90 days)
  • Current Drug Enforcement Administration (DEA) controlled substances registration
  • Minimum of 5 years of clinical experience in primary care, geriatric medicine, palliative care, or hospice medicine
  • Minimum of 3 years of medical leadership or director-level experience
  • Training and experience in the psychological and medical needs of homebound older adults living with chronic illness
  • Not excluded from participation in Medicare or other Federal Health Care Programs
  • Demonstrated ability to communicate effectively, both verbally and in writing
  • Valid driver's license and ability to travel within the region

Responsibilities

  • Clinical Practice (Approximately 60% of Time)
  • Lead by example through providing high-quality, evidence-based, patient-centered care to a small panel of patients.
  • Function as an independent medical provider in their capacity as a physician (internal medicine, family medicine, or related specialty such as geriatrics, palliative medicine) across all states within the designated region
  • Provide house calls (primary care in patient homes) and palliative care services for Ennoble patients in a designated geography near RCMO’s home
  • Deliver direct patient care including collection of medical histories, physical examination, differential diagnosis and clinical assessments, establishment of treatment regimens, and ongoing chronic disease management.
  • Effectively communicate with office staff and clinical operations across various internal platforms
  • Ensure timely completion of clinical documentation, diagnostic review, signing of home health orders, etc.
  • House call Medical Director Services
  • Clinical Direction: Oversee the provision of medical and administrative services by Company practitioners, including physicians, nurse practitioners, and physician assistants; provide clinical direction including regular review of medical charts; assist with development of guidelines and best practices for clinical activities
  • Supervision: Serve as collaborating or supervising physician for nurse practitioners and/or physician assistants as needed, pursuant to state regulatory requirements across the region
  • Clinical escalation: Serve as trusted consultant for providers requiring clinical escalation to review cases
  • Ensure continuity of care: during periods of provider vacancy, serve as the interim clinical point of accountability for unassigned patients, including oversight of safety checks, urgent issues, care coordination tasks, and alignment with organizational clinical standards.
  • Field Training / Clinical Assessments: Periodically provide direct clinical oversight in the form of field training and clinical assessments for new and/or underperforming providers
  • System Improvement: Assist Company to improve care delivery operational and clinical processes, including development of protocols and operating guidelines, training for clinical staff, and quality assurance activities
  • Backup Call Coverage: Be reasonably available for after-hours backup call coverage for escalations when primary call coverage is unresponsive or unavailable, as well as for clinical escalations for complex cases
  • Information Technology Product Guidance: Assist Company in suggesting, designing, and testing needs and features in Company's technology assets, including electronic health record (EHR) software.
  • Regional Leadership and Administration
  • Provide strategic medical leadership across assigned multi-state region
  • Coordinate with local clinical leadership to ensure consistent standards of care
  • Participate in performance improvement programs and quality initiatives
  • Support market growth initiatives including patient panel expansion and operational profitability targets
  • Collaborate with regional administrators and executive leadership on business development
  • Champion and support continuous improvement of core Ennoble programs such as BHI, Remote Patient Monitoring, GUIDE and ACO programs
  • Ensure compliance with Medicare/Medicaid requirements, Stark Law, Anti-Kickback Statute, and other applicable healthcare regulations across all regional jurisdictions
  • Represent Ennoble Care at regional professional conferences, meetings, and community events
