The Region Clinical Intake Specialist is responsible for managing and coordinating the clinical intake process for both Home Health and Hospice services across multiple branches. This role ensures timely, accurate, and compliant intake procedures using the Homecare Homebase (HCHB) system, serving as a key liaison between referral sources, clinical teams, and operational leadership. Performs all clinical aspects of the intake process, including communication with patients and families and admitting clinicians, clinical data entry accuracy, timeliness and Face to Face encounter accuracy and PCP/MD verification. Ensure patients are admitted timely and in accordance with the intake policy. Provides recommendations for process and policy improvements. Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements. Stays thoroughly informed about the details of clinical quality metrics of the organization has with key Home Care Leadership, healthcare networks, or other partners for Home Health and Hospice services. Ensures patient admissions comply with rules, requirements and terms in the contracts. Establishes and maintains positive working relationships with current and potential referral sources and associated clinical leadership teams. Provides a smooth coordination of care of patients to home care by overseeing patient education, care planning, and coordination with multiple providers, case managers and other clinical needs within established payer and financial guidelines. Strengthens and monitors community and customer perceptions of Hartford Healthcare at Home as a high-quality service provider, while collecting and reporting clinical referral data and key customer trends.

Requirements

  • Licensed clinician (RN, LPN, or equivalent) required.
  • 1 Year of Administrative/Clinical Healthcare Experience.
  • The Region Clinical Intake Specialist must have healthcare experience, preferably in referrals/intake in a home health or hospice environment.
  • Demonstrates good communication, negotiation, and public relations skills.
  • Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job responsibilities

Nice To Haves

  • Bachelor’s Degree Preferred (BSN, Healthcare Administration or related field) preferred.
  • 1-3 years of experience preferably in Home Care.
