About The Position

JOIN OUR TEAM AND MAKE A DIFFERENCE! Our clinical team is seeking a full-time, 40 hr./weekly or 36 hr./weekly REFERRAL NURSE or CMA at our Chandler Clinic. Schedule: M-F, 8:00 am - 5:00 pm or Monday 8am-12pm and Tuesday-Friday 8am-5pm. ECHO Community Healthcare is a Federally Qualified Healthcare Center that provides quality healthcare to all members of our community, including the insured, uninsured, under-insured and homeless. Job Summary Patients are referred to a specialist when the patient's needs are beyond the scope of practice for a the provider. The Referral Nurse appropriately and efficiently gathers needed information from external specialty services for established patients and internal referrals. The Referral Nurse works closely with Providers and Site Manager to evaluate patient satisfaction and outcomes of care and to provide coordination of services for patients within an integrated health care system. Demonstrates effective written and verbal communication skills

Schedule: Monday to Friday, Day shift
High school diploma/GED (preferred)
RN, LPN, or CMA license or certificate (preferred)
BLS Certification (preferred)
EPIC: 1 year (preferred)
1 year of clinical experience (preferred)
Spanish (preferred)

Responsibilities
