Summary: Referral coordinator facilitates retrieval of all medical and diagnostic reports and images for new referrals; verifies the accuracy of information obtained and schedules all new patient consults/evaluations.

Requirements

• High School Diploma or Equivalent (Required)
• Knowledge of Microsoft word and excel.
• Excellent communication skills, reliable, self-motivated and competent.
• Three (3) years of experience in a related healthcare field.

Responsibilities

• Download Compact Discs (CD) through the PACS scanning system and transfers images to patient chart.
• Work closely with nurse navigators, APRN's and physicians to ensure all patient information and documentation is obtained.
• Participate in special projects or activities as requested.
• Contacts patients and/or providers to obtain all pertinent medical records and verifies the accuracy of all information obtained.
