About The Position

At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems. Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks. This is a temporary opportunity for a limited period of time The Job The Referral Coordinator plays a crucial role in ensuring the smooth operation of various tasks related to patient data management, communication, tracking, and specialty referrals. The role involves meticulous data entry, efficient response to team inquiries, proactive tracking and monitoring, and active involvement in special projects as assigned. Requirements

  • High School Diploma or equivalent experience.
  • 1+ years call center or related customer service experience.
  • Previous experience in data entry and administrative tasks.
  • Strong communication skills, both written and verbal.
  • Detail-oriented with a commitment to accuracy in patient data management.
  • Ability to multitask and prioritize tasks effectively.

Nice To Haves

  • Experience in the healthcare, insurance or pharmacy industry highly desirable
  • Familiarity with medical record processes and specialty referrals is a plus.

Responsibilities

  • Create new patient charts and enter relevant information into designated platforms (Smartsheets, Canvas).
  • Review and update patient eligibility information.
  • Conduct audits on patient charts to ensure completeness and accuracy of enrollment forms.
  • Effectively respond to team requests and inquiries on multiple communication platforms (Teams, TigerConnect, Email).
  • Process medical record requests promptly and accurately.
  • Manage incoming faxes, assigning them appropriately to patient e-charts.
  • Monitor various communication platforms to ensure timely and appropriate staff responses.
  • Co-support the tracking of Cell Phone Distribution and Phone Line Reassignment.
  • Actively participate in team meetings, contributing insights, and following through on action items.
  • Act as a liaison between the requestor and specialty office.
  • Submit all required documentation for specialty referrals.
  • Identify new specialists to expand the network of specialty offices available to our patients.
  • Collaborate with the team and take on special projects or tasks as assigned, demonstrating adaptability and a proactive approach
