About The Position

The referral clerk is responsible for scheduling and tracking referral appointments to outside providers as well as making referrals and performing follow-up tracking for outside procedures. All Juniper Health employees will adhere to the Patient Center Medical Home (PCMH) model and work to ensure that patients of all ages receive comprehensive, coordinated and patient-centered whole patient care. The employee will maintain confidentiality at all times. ESSENTIAL FUNCTIONS: After ordered by a provider, schedules referral appointments AND referral procedures at a time and location that is as convenient to the patient as possible. Obtains pre-authorizations for all referrals as required. Prints appropriate, required referral paperwork, including required medical records, to: 1) give to the patient to transport to the referral provider, if required; 2) send via fax and/or electronic transfer to the referral provider. Updates the patients EMR with the appointment information. Ensures that all referrals are electronically transmitted to all referral sources that possess a secure, direct email address. Tracks ALL referrals to ensure that patients kept the referral appointment. If patient did not keep appointment, notify the provider to determine if referral appointment or procedure should be rescheduled. Obtains copies of referral appointment and referral procedure results from the source provider; Ensure that reports are correctly scanned into the patients medical record and are reviewed and signed off by a JHI provider. Per JHI tracking policy, makes at least 3 attempts to obtain referral reports. If unobtainable, notifies the provider via the patient messaging function within the EMR. Fill out LKLP transportation forms to assist patients with transportation. Ensure all information is accurate and signed off by the appropriate JHI personnel. Other duties as assigned. QualificationsWORK ENVIRONMENT: Work is typically performed in an office setting. PHYSICAL DEMANDS: The physical demands required of the position are representative of those required of comparable professional positions and must be met by the employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee is required to communicate, operate equipment, observe details at a close range, be in stationary or moving positions, and occasionally lift or move up to 25 pounds. TRAVEL: Travel to JHI sites and/or external meetings and trainings may be required. OTHER: The employee is expected to work and be available during regular business hours, however, extended hours and weekend work may be required.

Requirements

  • Education: High School Graduate or General Education Degree (GED): Required
  • Experience: Zero to two years of experience in a medical field or in a related/customer service area is required.
  • Computer Skills: Computer data entry, basic computer operating skills, Excel, Word, and EMR (Electronic Medical Record) preferred
  • Other: Excellent communication skills, accuracy
