About The Position

Fedcap NYC provides programs and services that change the lives of marginalized people facing barriers to economic and social wellbeing. Our Transition Planning & Transportation program (TPT) provides comprehensive, culturally competent services to individuals detained in city jails and supports their successful reentry into the community. As Reentry Mentor, you will be a core member of the Fedcap TPT team with responsibility for targeted outreach, engagement, and participant support for individuals housed in Rikers Island Department of Correction (DOC) facilities. You will be the first point of contact for individuals eligible for transition planning, helping guide them into services and maintaining consistent follow-up to promote their ongoing engagement. You will support communication, scheduling, and pre-release service connection, and may accompany individuals to community providers upon discharge.

Requirements

  • High School diploma or equivalent required, an associate degree or equivalent experience preferred.
  • Minimum of 1 year of experience in outreach, peer navigation, case support, or community engagement.
  • Lived experience of incarceration or justice involvement strongly preferred.
  • Experience working with justice-impacted individuals, public assistance recipients, and those affected by poverty.
  • Comfortable working within jail-based settings and engaging with diverse populations
  • Must pass the required NYC Department of Corrections security clearance to work on Rikers Island.

Responsibilities

  • Facilitate orientations in housing units to market program services to Persons in Custody (PICs)
  • Collaborate with the Department of Corrections staff for assignment of units to market to PICs newly incarcerated every week
  • Track PICs attendance in workshops and transition planning sessions to determine participants enrolled in program services
  • Follow up with participants to provide updates on requested housing referrals, documentation needs, or to deliver court letters and other supportive materials promptly.
  • Conduct outreach and engagement with 80% of assigned participants within 60 days of release date or with a scheduled court date.
  • Accompany and support participants during community transitions, ensuring ≥70% follow-through on scheduled connections.
  • Maintain 80% timely documentation of all sessions, outreach, and follow-up activity in FedcapCARES and DOC-tracking systems.
  • Participate in weekly team supervision and monthly partnership meetings to discuss progress, performance, and participant needs.
  • Collaborate with Transition Managers, Workshop Facilitators, and community partners to ensure seamless coordination for 80% of active cases.
  • Contribute to monthly data reporting and quality assurance reviews, identify areas for improvement, and ensure a ≤10% error rate in documentation.
  • Enter all workshop notes and outcomes into FedcapCARES within 48 hours of service delivery.
  • Maintain ≥ 80% data accuracy in participant records and documentation.
  • Ensure all data entry, reports, and case notes comply with DOC and Fedcap confidentiality and compliance
  • Maintain DOC clearance and comply with all facility and Fedcap policies regarding safety, conduct, and confidentiality.
  • Complete 100% of required DOC and Fedcap trainings annually and maintain compliance certifications.
