About The Position

The Re-entry Case Manager will play a critical role in supporting the successful reintegration of justice-impacted individuals. Reporting to the Director of Programs, the Case Manager will provide direct case management services to participants, addressing barriers to employment, housing, healthcare, education, and other essential areas. This role requires a trauma-informed, empathetic approach to service delivery and a deep understanding of the challenges faced by returning citizens. The Case Manager will serve as a consistent advocate and support system, helping participants navigate systems, access resources, and achieve self-defined goals.

Requirements

  • You have prior experience in case management, social work, re-entry services, or a related field (lived experience is valued and welcomed).
  • You are deeply committed to the dignity, rights, and empowerment of justice-involved individuals.
  • You are highly organized and able to manage multiple caseloads while maintaining empathy and professionalism.
  • You have strong verbal and written communication skills and are comfortable facilitating one-on-one and group interactions.
  • You are familiar with Washington, D.C.-based resources and systems relevant to returning citizens.
  • You are trauma-informed and culturally responsive in your approach to client engagement.
  • You are comfortable using case management databases and common software (Microsoft Office, Google Suite).

Responsibilities

  • Conduct Participant Intake & Assessment: Perform comprehensive intake assessments to understand participants’ goals, strengths, barriers, and service needs.
  • Develop Individualized Service Plans (ISPs): Co-create goal-oriented action plans with participants, focused on employment readiness, housing stability, mental health, substance use support, and personal development.
  • Provide Case Management & Support: Deliver ongoing case management, including referrals, appointment scheduling, transportation coordination, and check-ins to support goal attainment.
  • Facilitate Resource Navigation: Assist participants in accessing internal and external services, including public benefits, workforce development programs, legal aid, and more.
  • Track Progress & Maintain Documentation: Maintain accurate and timely documentation in accordance with organizational protocols and funder requirements.
  • Engage in Crisis Intervention: Respond to urgent needs and provide short-term crisis support while connecting participants with long-term resources.
  • Collaborate with Partners: Work closely with community providers, re-entry networks, housing organizations, and employers to ensure wraparound support.
  • Advocate for Participants: Serve as a strong advocate for participant needs within systems that include criminal justice, housing, employment, and healthcare.
  • Support Program Development: Provide feedback and insights to help evolve programming based on participant needs and trends.
