About The Position

The Case Manager will support the needs of patients in a new, 45-bed men’s behavioral health unit operated by Boston Medical Center (BMC) located in the Suffolk County Sheriff’s Department’s (SCSD) Jail. Serving as a liaison with patients to facilitate access to care and gathering necessary information to inform treatment decisions, the case manager will be a critical partner for clinical and administrative staff to provide intensive behavioral health services to incarcerated persons. They will assist patients in addressing unmet social factors as health such as housing, employment, state benefits and support with reentry needs including referrals to community based treatment. This role requires experience in navigating the substance use continuum of care. The role requires assertive outreach, advocacy, system navigation, and service coordination to manage barriers that may impede patients’ access to clinical services and progress in their recovery. Excellent interpersonal skills, patient engagement skills and the ability to work independently and collaboratively are key requirements of the job. Experience working with populations impacted by substance use disorders and significant mental illness is required as well as knowledge of relevant treatment systems. This position supports care teams within the Suffolk County Jail, work will be completed on site within a setting of incarceration. This is a full time position, work is primarily daytime, but candidate needs to be available for occasional evening and Saturday coverage. The person who fills this position must adhere to all of BMC’s RESPECT behavioral standards. As part of this innovative new collaboration between BMC and SCSD, this role will report into the BMC Department of Psychiatry.

Requirements

  • Bachelor’s Degree (or equivalent combination of education or specific certifications/licensures, which provides proficiency in the areas of responsibility, may be substituted for the education and experience requirements.)
  • Two to three years of experience working in social services environment
  • Ability to work collaboratively in a diverse, interdisciplinary team and the ability to adequately represent the needs of patients with behavioral health conditions and substance use disorders
  • Expertise in establishing working relationships with patients, their families, professionals, and the community
  • Knowledge of Boston area community, social, and health resources AND knowledge of the criminal legal system and resources specific to reentry
  • Sensitivity to ethnic, cultural, gender, and sexual orientation diversity, values, beliefs, and behaviors
  • Demonstrated ability to be flexible with patient’s priorities, evolving needs, and goals
  • Organizational skills to multi-task, set priorities, and efficiently complete assigned work
  • Effective interpersonal skills to facilitate communication with various members of the health care team, patients, and their families
  • Proficient oral and written English communication skills.
  • Intermediate Microsoft Office suite experience (i.e. MS Word, Excel, Access, Outlook)

Nice To Haves

  • Prior experience working in settings of incarceration or with a justice-involved population preferred.
  • Prior experience working with individuals impacted by substance use disorders and/or severe and persistent mental illness preferred.
  • Lived/personal experience is valued
  • Bi-lingual and/or bi-cultural candidates preferred
  • Experience using an electronic health record preferred

Responsibilities

  • Assess the case management needs of patients aligned with the Individualized Service Plans (ISPs) driven by clinical staff
  • Complete tasks related to behavioral health system navigation post-release, including but not limited to SUD and MH treatment referrals, primary care and specialty care needs.
  • Complete tasks related to connecting patients to unmet social needs, including but not limited to Employment/Workforce Development, Housing, State Benefits, Education, State agency coordination
  • Develop and maintain working relationships with internal SCSD staff, as well as external community service providers
  • Maintain current working knowledge of relevant resources and referral pathways including upkeep of resource list
  • Communicate effectively with all clinical and administrative leadership, ensuring that a patient’s care team is working in alignment
  • Assist in facilitation of skills building and reentry group sessions
  • Adhere to departmental/organizational policies and procedures including maintaining HIPAA standards and confidentiality of protected health information
  • Comply with established metrics for performance and adheres to documentation and work flow standards including clearly documenting all progress and submitting required data within expected time frames
