About The Position

The Recreation Specialist I plans, organizes, directs and participates in a comprehensive recreation program tailored to the needs and limitations of the frail elderly. Assesses the needs, interests and capabilities of participants and develops individualized recreation plans. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Develops and implements individual care plans that meet the interests, needs and capabilities of participants. Consults the recreation staff for the provision and implementation of recreation programs, individual and group, as specified in the care plan. Assessing and reassessing participants’ functional status in relationship to participation in recreational activities and implementation of 1:1 treatment plans. Leads and/or assists in large groups, small groups 1:1’s and individual activities such as exercise, arts and crafts, games, current events and sensory that meet the needs and capabilities of participants according to the care plan. Plans and develops the overall activity/recreation program for participants in collaboration with recreation assistants. Plans and schedules community field trips, outings and events. Performs preparation and cleanup of activities by putting away supplies and straightening out activity areas. Assists Day Center staff in providing lunch service and assistance with arriving participants, as appropriate and approved by the ADHC Supervisor. Orders supplies for department and maintains department equipment, within reason and ability of position. Completes assigned charting and reporting as assigned by Recreation Therapist. Oversees volunteers and senior companions assigned to the day center in areas such as monitoring services delivered, completing time cards/evaluations and assignments. Attends Care Plan Meetings for participants and consults with entire multidisciplinary team. Schedules entertainment and therapy for participants. Finalizes the Activity Calendar and orders/picks-up calendars for Day Center posting. Shops for activity supplies for different groups such as cooking and crafts, using personal transportation. Participates within the interdisciplinary team in the formulation of Plans of Care for InnovAge PACE program participants, as well as in other interdisciplinary team settings that plan, coordinate and monitor the care of InnovAge PACE program participants Additional work functions and duties may be assigned

Requirements

  • Recreation or Activity Coordinator with two years experience in a social or recreational program providing and coordinating services for a frail or elderly population within the last five years, one of which was full time in a patient recreation program in a Health Care setting.
  • A minimum of one year working with the frail or elderly
  • Bachelor’s Degree or equivalent work-related experience
  • Requires state issued driver’s license, personal transportation, good driving record and auto insurance as required by law

Nice To Haves

  • Current Certified Therapeutic Recreation Specialist (CTRS) certification
  • Bi-lingual-Spanish, Russian, Punjabi
