JOB DESCRIPTION: SUMMARY OF POSITION Responsible for collaborating with the client, multi-disciplinary team, support system and referral source on transition plan. Completes assessments upon intake. Works with clients to develop skill-building techniques needed for successful transition i.e. self-advocacy, problem solving skills, goal setting, communication, negotiation, symptom management and identifying triggers to prevent relapse, and community integration. Assist clients in overcoming barriers to recovery by coordinating/facilitating communication with various community systems. ESSENTIAL DUTIES & RESPONSIBILITIES Responsible for assessing clients’ needs upon intake and working with client through completion of goals including their recovery packet. Develops an accurate, comprehensive transition plan prior to treatment completion and shares with client, support system and referral source. Provides individual and/or group interventions to increase recovery capital and assist with transition planning. Assists clients throughout the process of applying for transitional housing, including helping them complete application and prepare for the interview. Transition plans are individualized but may include placement in a recovery house, connection to behavioral health care services such as group/ individual therapy, skills building, connection to recovery support groups, connection to primary care physician, and continuation of medication. Transition plans completed prior to client discharge and include the road map for recovery success once treatment is complete. Works closely with appropriate care team representative to ensure transition plans are added to the care plan and the continuing ASAM documentation. Uses structured standardized interventions and resources for teaching to assist clients in overcoming barriers to recovery. Works within various community systems to collaborate on behalf of, and with, the client. Includes the criminal justice system, the child welfare system, hospital/medical systems, educational, etc. Communicates client status with interdisciplinary team and referral sources including completing progress reports when necessary. Responsible for scheduling appointments, connecting clients with resources, and communicate transition plan effectively both client as well as referral sources.
