About The Position

We are recruiting compassionate and reliable Reablement Care Assistants to join our Welham Green Reablement team, supporting clients across Dacorum District (Hemel Hempstead, Tring, Berkhamsted, and surrounding villages), Hertford & Ware (SG12, SG13, SG14), and Sawbridgeworth & Surrounds (CM21, CM22). Reablement is a specialized, short-term, intensive service, typically lasting up to 6 weeks, designed to help individuals recover and relearn daily skills after events like a hospital stay, a fall, or an illness. Unlike traditional care, this role focuses on empowering clients by doing things *with* them, rather than *for* them. Care Assistants will encourage and physically support clients to perform daily tasks themselves, gradually stepping back as their strength and confidence improve. The service is highly goal-oriented, involving tracking progress, celebrating daily milestones, and ultimately helping clients regain the ability to live safely and independently without the need for long-term care. This role is ideal for individuals who wish to see the direct, life-changing results of their support.

Responsibilities

  • Rebuilding Confidence – patiently encouraging clients to try tasks themselves
  • Mobility & Physiotherapy Support – assisting with transfers, walking, and exercises prescribed by occupational therapists
  • Nutrition & Mealtimes – supporting them to safely prepare their own meals and drinks
  • Medication Management – prompting and guiding them to manage their own medication safely
  • Housekeeping Skills – helping them relearn how to manage light cleaning and laundry
  • Health Monitoring – closely observing progress and reporting improvements or setbacks
  • Social Inclusion – helping them regain the confidence to step back into their community
  • Documentation – logging specific milestones to track their journey to independence
