The duties of a Palliative Care, Post-Acute Nurse Practitioner, APRN mid-level provider include, but are not limited to, making rounds in the skilled nursing facilities, communicating to patients, facility staff members, providing accurate assessment, diagnosis, treatment, consultation, education and follow-up care to patients. MUST HAVE/QUALITY OF LIFE/ PALLIATIVE CARE EXPERIENCE. Monday-Friday and alternating Saturdays Quick Program Overview: Our program is grounded in the idea that quality of life reflects a person’s total well-being—physically, emotionally, and socially—including both positive and challenging elements. We provide care not just at end-of-life, but at any point where a patient’s symptoms, cognitive health, or day-to-day function are starting to impact their quality of life. We focus on early needs identification, collaborative care planning, and aligning treatment with each person’s goals. Key Services We Offer: 🧠 Cognitive Assessments We include formal cognitive screenings at intake to identify early decline, support communication, and ensure appropriate surrogate decision-making where needed. 🗂️ Goals of Care Conversations We guide patients and families through structured conversations about what matters most—making sure care aligns with values, preferences, and long-term priorities. 🏥 Discharge & Functional Planning We support transitions from hospitals or SNFs by helping families understand a patient’s new level of function and prepare for home care, outpatient follow-up, or other support services. Our team works closely with PCPs, specialists, and discharge planners to ensure care is person-centered, coordinated, and values-driven.
