This position is part of the downtown campus but the office is located at our HSSB building in North Liberty. Position Summary: The Home Health Quality Coordinator is responsible for oversight and coordination of all quality, compliance, and performance‑improvement activities within the Home Health program. This role ensures adherence to regulatory standards, supports optimal patient outcomes, and provides focused quality oversight and leadership to clinical staff. Key responsibilities include conducting chart audits, monitoring OASIS data integrity, facilitating interdisciplinary case reviews, coordinating utilization review, and providing onboarding support and ongoing education to clinicians. This position plays a central role in maintaining high standards of patient care and operational excellence.

Requirements

  • Master’s degree is required or equivalent to a combination of education and experience.
  • Current License to practice Nursing, Physical Therapy, Occupational Therapy, or Speech Therapy
  • Minimum 1 year of related experience within quality coordination, process improvement, or home health care operations
  • Prior supervisory experience within quality coordination, process improvement, or home health care operations
  • Demonstrates leadership and training skills including strong clinical skills
  • Excellent written, verbal, and interpersonal communication skills
  • Experience with clinical documentation review and compliance

Responsibilities

  • Assist with development of performance improvement priorities and agency-wide quality initiatives.
  • Implement, monitor, and evaluate quality, compliance, and utilization activities.
  • Conduct detailed reviews of medical records—including plans of care, clinical notes, and progress notes—to ensure accuracy and compliance.
  • Monitor OASIS data integrity and complete required OASIS submissions.
  • Ensure accuracy of all publicly reportable data and quality metrics.
  • Produce quality reports to evaluate services delivered and identify trends or areas of opportunity.
  • Facilitate interdisciplinary case conferences and monitor coordination of care.
  • Review, maintain, and update clinical policies and procedures in partnership with leadership.
  • Assist with onboarding, training, and educational needs assessments of home health staff.
  • Provide ongoing education to clinical staff as a group and individually to ensure compliance and high-quality patient care.
  • Support clinical competency evaluations to ensure high‑quality patient care.
  • Direct supervision of clinical staff.
  • Support departmental customer service and financial goals.
  • Assist department leadership with operational tasks as needed.
  • Complete required electronic medical record tasks and contribute to workflow enhancements.
  • Assist with coordination of clinical activities of Home Health staff, whether hospital-based or contracted, who are actively engaged in providing home health services.
  • Serves on Home Health committees.
  • Assist Administrator with program compliance with applicable laws, regulations, and accreditation standards and in keeping with hospital and medical staff policies.
  • Coordinates and facilitates patient care across the care continuum and home care.
  • Assures patient needs are continuously assessed.
  • Utilizes standards of care and practice guidelines to plan, implement, and evaluate patient care needs.
  • Assures the development, implementation, and update of the individualized plan of care.
