We're are looking for a detail-driven Quality Assurance (QA) Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a passion for accuracy, ensuring compliance, and supporting our care teams. This role plays a critical part in protecting patients, clinicians, and the agency while driving continuous improvement across the organization. Quality Assurance RN/ LPN responsibilities include: Conduct systematic reviews and audits of clinical documentation, including OASIS assessments, visit notes, care plans, and discharge records, to ensure accuracy, completeness, and timeliness. Monitor compliance with Medicare Conditions of Participation, CMS guidelines, accreditation standards, HIPPA and state regulations applicable to home health services. Identify documentation deficiencies, trends and risk areas; collaborate with clinicians and leadership to develop and monitor corrective action plans. Provide education, coaching, and feedback to clinical staff on documentation standards, OASIS accuracy, clinical best practices, infection control, and regulatory requirements. Review incident reports, adverse events, and patient complaints; participate in root cause analysis and quality improvement initiatives. Support survey readiness activities, mock surveys, and ongoing quality monitoring to minimize deficiencies and denials. Analyze quality indicators and performance data to identify opportunities for improvement in patient care delivery and outcomes Maintain up-to-date knowledge of regulatory changes, ICD-10 coding updates, and evolving best practices in home health care.
