About The Position

"Navitas Healthcare, LLC" is seeking an experienced Quality Assurance Nurse (LPN) to lead quality initiatives, staff education, and infection prevention activities in a long-term care or nursing facility setting. This role combines Quality Assurance, Staff Development, and Infection Prevention responsibilities, working closely with leadership to ensure regulatory compliance and high standards of resident care.

Requirements

  • Current LPN license in Arizona
  • Current Fingerprint Clearance Card
  • Infection Preventionist Certification
  • Valid Arizona Driver’s License for in-state travel
  • Strong verbal and written communication
  • Problem-solving and critical thinking
  • Data analysis and interpretation
  • Ability to develop and coordinate training programs
  • Knowledge of nursing home regulations, quality assurance methods, and adult learning principles

Responsibilities

  • Implement and maintain policies and procedures related to quality assurance, facility practices, and regulatory requirements.
  • Gather and analyze data through audits, interviews, and observations to monitor compliance across departments.
  • Review reports and develop QAPI-based Performance Improvement Projects (PIPs) as needed.
  • Prepare and distribute reports, data summaries, and statistical analyses related to facility performance.
  • Assist supervisors in developing corrective action plans and monitor progress.
  • Investigate incidents of non-compliance and collaborate with leadership on allegations of abuse or neglect.
  • Coordinate facility preparation for regulatory surveys and support surveyors.
  • Chair and coordinate internal Quality Assurance and Infection Control Committee meetings.
  • Identify staff training needs and develop, implement, and evaluate educational programs.
  • Assess training effectiveness to ensure staff meet expected outcomes.
  • Maintain accurate training records for regulatory compliance.
  • Serve as the facility’s Infection Preventionist.
  • Track and analyze infections using accepted criteria for surveillance and trending.
  • Conduct daily infection control rounds to confirm compliance with care plans and prevention practices.
  • Stay current with CDC, CMS, and other applicable guidance.
