About The Position

The Front Door Diversion Quality Assurance & Care Coordinator is a member of Thresholds Intake Department. The focus of the team is on member satisfaction, service efficacy and availability and member outcomes. The team ensures that funder expectations are being met in service delivery and are available to offer support, guidance and deliver hands‐on training for improvement. The Front Door Diversion Quality Assurance and Care Coordinator serves as the liaison between the agency, IDHS and subsidy administrators. The Quality Assurance and Care Coordinator monitors service delivery and outcomes by maintaining frequent contact with the community service providers, the Thresholds treatment teams, subsidy administrators and direct contact with the members. This role is responsible for the reporting of member transition status and outcomes to all parties involved.

Requirements

  • Master's and/or doctoral degree in counseling, social work, or a closely related field required.
  • Excellent planning, organizing and team‐building skills.
  • Ability to work effectively with staff at all levels of the organization.
  • Ability to function in a computer-oriented office, including working knowledge of pertinent software.
  • LPC or LSW required.
  • Valid driver's license and daily access to a car is required. Willingness to provide transportation to members required.

Nice To Haves

  • LCPC or LCSW preferred.

Responsibilities

  • Responsible for in‐depth review and evaluation of operational systems, documentation and service delivery according to the expectations and regulations of the various funding sources and with the highest professional standards. This may include, but is not limited to, the standards and documentation requirements as established by: CARF, Medicare, Medicaid, IDHS, DMH, DOA, CILA, DOL, DORS and/ or DCFS.
  • Identifies areas for improvement and in collaboration with the Program Director, develops actions plans.
  • Makes recommendations regarding additional training needed for staff. Provides direct hands-on training.
  • Responsible for helping Programs create and enhance a therapeutic environment.
  • Attends and participates in task force meetings and other agency‐wide committees as assigned.
  • Monitor service delivery and availability of needed supports through direct contact with the member and ensures that all basic needs are being met. This requires face to face meetings with member or guardian, treatment teams and external collateral to identify goals, objectives, specific mental health services needed and identification of staff responsible for delivering the services.
  • Advises direct service staff regarding appropriate responses to members' needs.
  • Develops criteria by which to review the delivery of all departmental services and make recommendations to assure they operate using the values and principles of recovery.
  • Provides accurate and timely information to superiors on all topics and issues related to the operation of the agency and the wellbeing of its staff and members.
  • Oversee weekly reporting for submission to IDHS.
  • Document Thresholds and IDHS Quality Indicators across transition. Create and complete agency reports for submission to IDHS and the Court.
  • Acts as an additional clinical eye and facilitates communication between all internal and external treatment providers and supports.
  • Participates in Critical Case Review Panel calls with the state and completes needed follow up in collaboration with the Program Director.
  • Other duties as assigned.
  • May be required to participate in crisis intervention when necessary.
  • May be required to attend various trainings developed by funders.
  • May be required to supervise graduate students at the request of Program Directors or other supervisors as appropriate.
  • May be required to provide other clinical services as needed to facilitate the quality of services across programs.
