About The Position

Homeward Bound, a mental health and substance use facility for adults, is hiring a Qualified Mental Health Professional to work on the crisis residential unit where clients reside for 7 days or more while they get stabilized due to being severely depressed or suicidal, homicidal towards specific individuals, or psychotic without the acuity of needing a locked down facility. Candidates must have a minimum of a bachelor's level education in psychology, counseling, sociology, social work, or related field.

  • Bilingual in Spanish and English is a plus.

Responsibilities

  • Complete an initial assessment on each client within 24 hours of arrival on the crisis unit.
  • Assist clients with all case management needs including, but not limited to, appropriate housing upon discharge, connection with positive social support, and coordination with necessary resources.
  • Coordinate client care between crisis residential and mental health outpatient and other aftercare providers.
  • Complete intake documentation with clients including financial information.
  • Prepare clients in crisis residential for discharge and work with the treatment team to eliminate barriers to success.
  • Create person-centered treatment plans with individualized goals, objectives, and strategies.
  • Assist in direct client care on the unit including observation, checking in and out belongings, facilitating client phone calls, and attending meals.
  • Facilitate psychoeducational groups daily and complete documentation.
  • Provide therapeutic intervention in potential crisis situations and assist the team in ensuring the appropriate level of care and coordination of services is provided.
  • Attend all treatment team staff meetings.
  • Perform a daily evaluation and documentation of each client’s mental status including assessing for suicidal ideation, homicidal ideation, and audio/visual hallucinations.
  • Assist in clinical decision making regarding both admissions and discharges.
  • Be responsible for scheduling and maintaining communication with both residential and outpatient clients.
  • Be familiar with different funding sources and the documentation that each requires.
