The primary purpose of this position is to provides services for adults diagnosed with severe mental health issues that may make it difficult to accomplish daily tasks. Work as part of a collaborative process to assess, plan, implement, coordinate, monitor, and evaluate the options and services required to meet an individual's health and human service needs. Provide treatment services which provide individual case management, medication assistance, skill building, groups, and a variety of socialization activities. Specific to Open Door case management are service planning and coordination, family and social relationship building, referral to substance abuse treatment and support, housing assistance, referral to supported employment and education, wellness self-management, problem solving, skills development, (including basic personal care and safety skills, grocery shopping and cooking, purchasing and caring for clothing, household chores, using transportations, assessing other community resources), health education and support, medications access, financial management, and empowerment and self-help. Support and assist clients with OHP enrollment and coordinating maintained access to OHP benefits.
