About The Position

Options for Southern Oregon is a Certified Community Behavioral Health Clinic (CCBHC) that specializes in providing services that are both trauma-informed and recovery-oriented to persons/consumers with mental illness and addictions. Trauma-informed care includes safety, choice, collaboration, trustworthiness, and empowerment for clients. The Care Coordinator is a healthcare professional who will be responsible for facilitating communication and collaboration with healthcare providers from emergency departments, hospitals, and other acute or sub-acute, or residential placements to ensure access to services, a seamless transition of care, and optimal health outcomes for our clients. The Care Coordinator will have a strong understanding of the healthcare system, including hospital operations and patient care protocols.

Requirements

  • Bachelor’s (BA/BS) Degree in a human services field OR three years of experience providing services in a similar setting with individuals who have emotional and/or mental disabilities and/or developmental disabilities, OR a combination of education and experience that provides the incumbent with the desired skills, knowledge, and abilities required to perform the job.
  • Must be a QMHA on the Mental Health & Addictions Certification Board of Oregon (MHACBO) registry.

  • Experience working with clients with mental illness.
  • Previous experience in care coordination, case management, or a similar role within a healthcare setting.
  • Possess business experience sufficient to meet the demands of the position, including proficiency with a variety of software applications.
  • Must pass state-required background and DMV checks; possess a valid OR driver's license and a satisfactory driving record.

  • Bilingual skills a plus; Options pays a premium for employees who can conduct business in Spanish and provide certification of a language proficiency test approved by the Oregon Health Authority that meets a proficiency level of “High Intermediate”.

Responsibilities

  • Coordinates client care activities among multiple healthcare providers, both externally and internally.
  • Serves as a liaison between clients and hospitals, ensuring that patients receive timely and appropriate follow up services and connections to resources upon discharge.
  • Schedules discharge appointments and makes appropriate referrals within agency resources.
  • Tracks client progress upon discharge to ensure engagement in services. Work to remove barriers to care.
  • Works with treatment teams to modify service plans to address current client situation.
  • Monitors client progress during hospitalizations by participating in treatment team meetings and reading relevant documentation.
  • Provides education and resources to patients and families regarding available services and care options.
  • Facilitates discharge planning for clients transitioning from the hospital to home or other care settings.
  • Maintains accurate and confidential patient records, documenting care coordination activities and patient status.
  • Collaborates with a multidisciplinary team to evaluate and improve care coordination processes.
