Job Summary: The Public Health Programs Coordinator plays a key role in advancing the mission of the Family Resource Network’s Public Health Programs department. This position provides high-quality, person-centered Information & Referral (I&R) services across phone, email, virtual and in-person modalities. The Coordinator will support individuals and families navigating New Jersey’s complex aging, disability, and health systems, with a particular emphasis on services related to intellectual and developmental disabilities (IDD). In addition to direct services, this role helps expand the agency’s presence through community outreach, health education initiatives, webinar development, caregiver programming, and emerging work at the intersections of nutrition, access to care, aging and disability. This is a dynamic position reporting to the Director of Public Health Programs ideal for someone who is community-oriented, creative, and able to manage projects from concept to completion with minimal oversight. This role is primarily remote with in-office (2 times per month) and community presence (1 time per week) requirements.

Requirements

  • Bachelor’s degree in Public Health, Human Services, Social Work, Health Education, Disability Studies, or related field.
  • Bilingual proficiency, Spanish preferred.
  • Experience working with individuals with disabilities, aging adults, caregivers, or similar populations.
  • Knowledge of New Jersey’s IDD systems and community resources or willingness to learn.
  • Strong communication skills with ability to explain complex systems clearly.
  • Ability to manage projects independently.
  • Comfort with presenting, community engagement, and direct service delivery.
  • Strong writing and organizational skills; proficiency with virtual platforms.

Nice To Haves

  • CHES, MCHES or CPH certification.

Responsibilities

  • Provide culturally competent, trauma‑informed, and accurate I&R services by phone, email, virtually and in-person.
  • Assist individuals and families in navigating the IDD system, aging services, disability programs, Medicaid/DDD/SSI benefits, and community-based supports in New Jersey.
  • Maintain knowledge of state and local resources, service systems, eligibility criteria, and care pathways.
  • Document all interactions in the agency’s database in compliance with program requirements.
  • Identify emerging consumer needs and share insights with program leadership.
  • Represent the organization at community events, health fairs, trainings, caregiver programs, and conferences.
  • Deliver presentations or resource overviews to community groups, professionals, and families.
  • Strengthen community partnerships and promote awareness of FRN and affiliate services.
  • Assist in planning and delivering health education webinars and workshops.
  • Brainstorm and execute educational content that is accessible and inclusive.
  • Manage webinar workflow from topic development through post-event follow-up.
  • Provide basic guidance on health insurance options, including Medicaid, Medicare, commercial insurance, and Marketplace plans.
  • Support consumers during Open Enrollment and Special Enrollment Periods through the Get Covered NJ platform.
  • Maintain health insurance navigator certification through Get Covered NJ.
  • Participate in planning and coordination of the National Caregivers Conference.
  • Assist with caregiver-focused initiatives and events throughout the year.
  • Stay informed about caregiver services and resources in New Jersey and nationally.
  • Contribute to the growth of the Center on Nutrition and Disability.
  • Assist with outreach, educational content, and program development.
  • Maintain interest in the intersection of nutrition, disability, aging and public health.
