Requirements

  • Applicants should possess a Master’s degree in psychology or related field.

Responsibilities

  • Assessing patients from a biopsychosocial perspective to determine care needs including psychological, financial, and medical interventions.
  • Providing primary care and behavioral medicine psychological interventions and/or assessment services for patients when needed/requested.
  • Developing patient focused treatment plans and monitoring success towards goals.
  • Coordinating and referring care services between patient and provider
  • Preparing and maintaining confidential patient records, including case management notes.
  • Attending monthly Ryan White clinic at Family Medical Center.
  • Providing HIV health education and risk reduction to patients and their partners on a routine basis.
  • Makes referrals to various community resources (Federal, State, Local and others as appropriate) to meet patients’ needs.
  • Attends training courses, as mandated by the funding agency.
  • Delivering client advocacy and coordinating client housing needs.
  • Facilitating peer-led support groups, when program Coordinator is absent.
  • Adhering to HIPPAA and ACT148 regulations.
  • Conducts pre-surgical psychological evaluations for bariatric surgery patients and provide follow-up supportive therapy for these patients as needed or requested.
  • Provide individual psychotherapy services for children and adult Family Medical Centers’ patients as needed or requested as part of the Family Practice Residency Program’s integrated approach to patient care.
  • Interacts with attending physicians, physician advisor and department chairs about any identified quality issues and utilization issues.
  • Participates in patient care, resident training, and research activities as those needs arise in the Family Practice Residency Program.
  • Identifies and reports barriers that impede appropriate levels of care for trending purposes; immediately intervenes with corrective action based on barrier identified.
  • Collects data pertinent to Case Management processes and utilizes results to seek data-driven performance improvement objectives.
  • Performs retrospective chart reviews as necessary for authorizations/payment.
  • Provides crisis intervention as necessary. Crisis intervention may include, but is not limited to, suspected abuse cases, deaths, codes, children/youth issues, and terminal illness.
