About The Position

We are seeking for a compassionate, motivated, and clinically skilled PMHNP to support our growing behavioral health services in nursing homes throughout Niagara County, NY. Requirements

  • Active New York State Nurse Practitioner license with PMHNP certification.
  • Ability to work independently while effectively managing a varied caseload.
  • Strong clinical assessment, diagnostic, and medication management skills.
  • Excellent communication and interpersonal abilities with the capacity to build trust and rapport with patients and staff.
  • Proficiency in EHR systems and basic computer applications.
  • A compassionate, patient -centered approach to psychiatric care.

Responsibilities

  • Conduct in -depth psychiatric assessments and evaluations for residents within skilled nursing and long -term care facilities.
  • Identify mental health conditions and create individualized, evidence -based treatment plans tailored to each patient.
  • Manage and adjust psychiatric medications while monitoring patient progress and response.
  • Work collaboratively with an interdisciplinary care team—including psychologists, physicians, nurses, and social workers—to ensure coordinated and comprehensive treatment.
  • Provide therapeutic support, including counseling and psychotherapy, as appropriate.
  • Educate residents and their families about diagnoses, medication use, treatment goals, and strategies for maintaining mental wellness.
  • Maintain accurate, timely, and compliant documentation within the electronic health record system.Participate in case discussions, interdisciplinary meetings, and quality improvement projects aimed at enhancing patient care
