About The Position

Join our team and be a part of something extraordinary as part of Bridgeway’s Community Support Services. As part of the core services team, the PACT RN provides nursing assessment, direct-support services and primary health care to seriously and persistently mentally ill individuals enrolled in the PACT Program. Provides support and education to other team members in nursing/medical treatment and health care. Psychiatric RNs on PACT teams are critical to the success of the people served by Bridgeway. RNs provide nursing assessment, direct-support services, and primary health screening to individuals whose goals include recovery from mental illness and substance use conditions. PACT RNs use their skills to provide leadership, support, and education to other team members in nursing/medical treatment and health care. They are an essential part of a multi-disciplinary team working together to provide individuals the tools to become and remain stable and healthy. PACT RNs receive ongoing training, and supportive supervision as they deliver evidence-based interventions, and wrap-around services. You will be taking vitals, drawing blood, injections and assessments.

Requirements

  • Bachelor’s degree in nursing preferred but Associates in nursing accepted
  • Valid NJ Registered Nurse license
  • Valid driver’s license required; no more than one moving violation within the past 12 months

Responsibilities

  • Participates in frequent monitoring and assessment of the mental and physical health status of people serves.
  • Works with the team on nursing assessments, referrals, individual recovery plans, and population health including medical needs, tracking medication, screenings and recommendations (PSA, mammograms, colonoscopy, etc.).
  • Participate in clinical and non-clinical team meetings.
  • Coordination of services with other community mental health and non-mental health providers.
  • Educate people served and family/significant others on mental and physical health, medication (focusing on psychiatric medications), side-effects, symptom management and physical health care.
  • 24-hour on-call coverage on a rotating schedule, with the ability to respond immediately to individuals by phone or in person.
  • Crisis response and accompanying the individual to psychiatric screening/assessment center or emergency service.
  • Administers prescribed medications and injections, draws blood, and prepares laboratory specimens.
  • Train other team members regarding medication education, medication delivery observation of self-administration of medication, and medication monitoring; and regularly assess other team members’ competency in this area.
  • Assess individuals’ daily living skills, and teach individuals needed skills in the areas of budgeting, cooking, laundry, cleaning, shopping, personal hygiene, community transportation, self-advocacy and ways to maintain housing.
