Requirements

• Prior experience as a PRSC or mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility is strongly preferred
• Bachelor’s Degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology) and have a minimum of 1 year of supervised experience in mental health or human services.

Nice To Haves

• Experience with Point Click Care (PCC) is ideal, but not required

Responsibilities

• Provide group and individual psychiatric rehabilitation services
• Deliver case management support to adults with a history of psychiatric hospitalizations
• Help residents build daily living, coping, and social skills
• Develop and carry out therapeutic interventions focused on long-term stability
• Collaborate with an interdisciplinary care team to support individualized recovery plans
• Document services and progress according to program and regulatory requirements
